Hospital in the home (HiTH) provides hospital standard acute care by a multidisciplinary team of nurses and allied health professionals in the comfort of your own home for specific conditions. HiTH is a safe alternative to hospital admission for conditions such as;
- Community Acquired Pneumonia / Chest Infection
- Exacerbation of COPD / Bronchiectasis
- Urinary Tract Infection / Pyelonephritis
- DVT (Deep Venous Thrombosis)
- Iron Deficiency Anaemia
Specialist Registered Nurses, Physiotherapists, Occupational Therapists, Social Workers, Pharmacists & Community Care Aides can visit you daily to provide comprehensive care that would otherwise require a hospital stay. Infections that do not improve with oral antibiotics and require treatment with intravenous antibiotics (or other intravenous medications) can be administered. Assessment and planning for social and domestic support to enable independent living at home, access to Tanspac and Compacks for those assessed as eligible can be arranged.
Referral is available from participating general practitioners (GPs), so ask your GP if 'Hospital in the Home' is an option of care suitable for you. APAC can provide this service 7 days per week including public holidays whether you live at home or in aged care anywhere within the Northern Sydney Local Health District and the service is free.
